1. Renal hypoperfusion

Fluid resuscitation can effectively prevent acute renal failure. The primary mechanism that can facilitate this is the renal hypoperfusion or pre-renal azotemia. This pathophysiologic framework, however, is not recognized in patients who are critically ill. Acute renal failure commonly occurs with many mechanisms that include hypovolemia and different varieties of shock. Even sepsis and accidents can occur if the renal hypoperfusion is coupled with the release of endogenous nephrotoxins. In a war field scenario, aggressive fluid resuscitation to injured soldiers may reduce the chances of acute renal failure but it may lead to sepsis causing further problems, leading to a worsening of the case. This clearly infers that hemodynamics can be marked satisfactory only if an enhanced resuscitation of organs of the renal system does not lead to any rapid changes. However, it has a quick action leading to saving lives of accident victims by slowing renal failure.

2. Crystalloids and colloids intake

Fluid resuscitation can be a major result for patients with renal hypoperfusion due to absolute or relative hypovolemia occurring due to the reduced renal perfusion pressure, like in cases of sepsis, organ failure or even slower cardiac output. In these cases, only fluid administration does not help to a great extent. In these cases, crystalloids and colloids are the basic fluids used. These crystalloids are small electrolytes with practically no oncotic properties. Crystalloids work with the principle of sodium balance and water retention. They can pass through the cell membranes and capillary membranes that make the procedure risky. However, colloids are larger molecule that stays in blood for a long time. The expansion effect of these compounds and colloids makes them useful for prevention of renal failure. This procedure acts very quickly but needs medical expertise for the dosage.ย 

3. Vasoactive drugs

Treatment of hypotension is a necessary precaution to prevent acute renal failures. Regular hypotension is regarded as below 65 mm Hg. Constant fluid resuscitation or optimization of intravascular volume with shock therapy to patients cannot rule out renal insufficiency. Vasopressors are used to increase MAP and are effective in preventing acute renal failure in patients suffering from hypotension. These drugs are needed to be taken regularly to regulate the proper pressure in the system and proper kidney functioning as they help in renal perfusion along with cardiac infusion. However, there are no regular clinical dosages available to patients. They need to be properly delivered only after correct dosage is determined after titration and taking consideration of MAP and other clinical renal perfusion data.ย 

4. Drugs to prevent contrast induced nephropathy

Contrast induced nephropathy is a tough and serious condition that arises due to intravenous infusion of radio contrast media resulting in increased creatinine concentration. These conditions are very rare. These drugs prevent acute renal failure resulting from prolonged and untreated nephropathy. These medications should be taken only on medical advice. However, frequent urine testing can result in rapid detection of nephropathy. These drugs are mostly dopamine or calcium brokers. However, these drugs are harmful for the body, so proper fluid administration is highly recommended. N-acetylcysteine is found useful in most cases. The process is effective in prevention of renal failure; however, reduced fluid intake can result in other diseases.ย 

5. Protocolized intravenous fluid administration

The process is the best for treating renal insufficiency and nephropathy. Sometimes infusion can work better alone but in rare cases it may be coupled with drugs to cure nephropathy. However, the process is wonderful for patients who have minor renal deficiency. Lesser urination, burning sensation during urine discharge and yellowish color can be regarded as first stages of renal insufficiency. Proper medical care is needed but the whole insufficiency can be reversed if detected early. Sodium carbonate along with saline can be useful in chronic cases but it should always be under medical supervision. It does not have any negative effect if properly administered.ย 

6. Avoid unnecessary anti-infectious medications

People in countries with improper drug sale regulation often end up taking wrong medications following a self-diagnosis. These drugs are often taken without any need and in wrong dosages. In ICU, these drugs are pumped in to prevent further infection. Almost all these antibiotics, antiviral and anti-infectious compounds can cause nephropathy leading to acute renal failure, which is as high as in 20% of cases. Therefore, if they are not medically advised, one should always avoid taking them.ย 

7. Lifestyle changes with high fluid intake

This strategy is not medically authenticated but it is the only way to ensure proper functioning of renal system. With proper fluid intake, one always flushes out the toxins, prevents formation of oxalate crystals inside kidney and urinary system. It also flushes out the unnecessary drugs out of the system. However, merely drinking lot of water once a day does not ensure a cure, it needs to be incorporated in regular lifestyle for rapid prevention of disease. Drinking refreshing mineral beverages have a positive effect on renal system. Therefore, every hard strenuous exercise and events should be followed with a tall glass of water with lots of essential minerals for proper functioning of the whole system.ย
